Perforating and/or Penetrating Chest Trauma due to Bullet of Shrapnell

Abstract
This report consist of clinical review of 150 cases of chest trauma due to gunshot or shrapnell,
seen and treated at the Dept. of General Surgery in Capital Army Hospital, Seoul, During past 10
years from Aug. 1960 to July, 1970.
1. 150 cases consist of 101 cases(67.3%) of Perforating wound and 49 cases (33.6%) of Penetra
ting wound, and perforating gunshot wound predominated over penetrating wound by the ratio of
2:1.
2. 150 cases (100%) were treated with closed thoracotomy in earlt postinjury period, 13 cases
(8.66%) with open thoracotomy, 24 cases (16%) with repeated thoracentesis. Tracheostomy was
perforemed for 11 cases (7.3%).
3. Post-operative complication was noted in 54 cases (36%). The most common complication
was wound infection, (13 cases; 24.07%) and the next was atelectasis and pneumonia(each 11
cases; 20.37%).
4. Overall mortality was 5.4% and principal cause of death was hemorrhagic shock( 2cases ; 25%)
incident to trauma and associated with massive thoraco-abdominal injury (3 cases; 37.5%) secondary
to acute cardiopulmonary insufficiency and there were treated conservatively without significant
morbidity.
5. It is proposed that unless there are clear-cut indications for the open thoracotomy in chest injury,
treatment should be directed toward meticulous supportive therapy and closed thoracotomy. Indication
for the open thoracotomy has been discussed/
6. Regarding above mentioned datas closed thoracotomy is thought to be emergency life saving
procedures as well as curative procedures in perforating and penetrating chest trauma.
